Diabetes Programs
Getting Started: What is Pre-Diabetes?

Join a registered nurse who’ll help you better understand:
• pre-diabetes;
• diabetes;
• risk factors;
• diagnosis; and
• the important management strategies for a healthy life.

Diabetes Wellness Series (part A and B)
November 28, 30 • 6-8:30pm or
February 27,29 • 6-8:30pm
Learn from a variety of diabetes specialists.
Connect with others living with diabetes.
Take action with practical tips for healthy eating, self-managing and physical activity

Next Level ‘Carb Counting’ for Diabetes
Management
Tuesday December 5 • 6-8:00pm
Our registered dietitian can show you how to identify and balance your carbohydrate intake to improve your blood sugar control. We recommend that participants already have basic diabetes knowledge or have participated in the Diabetes Wellness Program.

Diabetes Management – Insulin 101
Tuesday March 5 • 6-8:00 pm
Are you using or discussing the use of insulin as part of your diabetes management?
Join us to better understand the safe and effective use of various types of insulin.
By participating you’ll:
• increase your confidence in insulin use;
• connect with peers; and
• get answers from a diabetes specialist.

We recommend that participants already have basic diabetes knowledge or have participated in the Diabetes Wellness Program. In-person attendance highly recommended because of hands on activities involved.

Exercise Programs
CLA:D Program
October 10-November 24 1-2:30pm, in person at the CCSP office
This 7-week education and exercise program is for people with stiff and/or painful knees and/or hips, or people with
knee and/or hip osteoarthritis. It provides education and targeted exercise that can be applied to movement in everyday activities. The program is suitable for individuals with early to late stages of Osteoarthritis. This program is offered in partnership with PhysioPlus.

Self-Management Programs @
Your Health Your Way
On-going enrollment; only offered virtually over Zoom

This 6-week, group program will help you to: better manage your symptoms; learn self-management skills; and reach goals by taking small steps and discover tips to manage day-to-day activities.
You’ll also receive support from others with long-term health concerns and be able to share your experiences.

Heart Programs
Living Well with Heart Failure
October 25 • 10-12:30pm
Join a registered nurse, dietitian and pharmacist for this group education session on heart failure management. The presenters will outline what heart failure is and how you can manage it through:
• self-monitoring;
• medications;
• healthy eating; and
• other healthy lifestyle practices.
