WAHA: How to get your COVID-19 Vaccination Receipt

I have a GREEN Ontario Health Card:
- Visit the Ontario Vaccination Service and use your health card to download your receipt: https://covid19.ontariohealth.ca/ OR
- Call the Ontario Vaccine Booking Line at 1-833-943-3900 for support accessing your vaccine receipt.
I have a RED & WHITE Ontario Health Card
- Call the Ontario Vaccine Booking Line at 1-833-943-3900 for support getting your vaccine receipt
I do not have an Ontario Health Card
(Use this method if you have an out-of-province health card, or no health card at all)
- STEP 1: Get a COVID-IDComplete our WAHA COVID-19 Vaccine Receipt Support Survey to indicate that you need a COVID-ID: https://forms.office.com/r/Y4xXnNzRVJ
- STEP 2: Get your receipt using your COVID-IDfrom the special COVID-ID website: https://covid19.ontariohealth.ca/receipt-identity ORCall the Ontario Vaccine Booking Line at 1-833-943-3900
I tried these options but still couldn’t get a receipt
