Position Summary

Come enjoy stunning scenery, a mild climate and small town life in beautiful Creston, BC! We are seeking a Home Health Registered Nurse who wants to make a difference in the lives of others in a community setting.

This position will work primarily in the outpatient/ambulatory care clinic and infusion clinic. This position provides both direct client care and care management services.

Your Home Health nursing practice supports the client’s achievement of safe realistic and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. Working in collaboration with the client, family and other care providers you will develop, implement and adapt a care plan, deliver direct care, assess service eligibility and provide care management to help clients manage their own care and navigate through the various services available within the community setting.

Shifts are rotating and shift times are 08:30 to 16:30.

Graduation from an approved school of nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).

Candidates must also hold a valid B.C. driver’s license.
