Ontario is quietly introducing a new intensive home care initiative aimed at helping patients transition out of hospitals more quickly — a move designed to relieve mounting hospital overcrowding across the province.
The program, called High-Intensity Bundled Home Care, will provide hospital-level care at home for patients who no longer need acute medical treatment but still require significant support. Most are seniors or patients waiting for long-term care placement who currently occupy hospital beds because no suitable alternative setting is available.
Ontario Health atHome, the provincial agency overseeing community-based care, has asked home care providers to submit proposals to deliver the new service. According to internal documents obtained by CTV News, the request for proposals went out Nov. 7 and the program is expected to begin Dec. 8 in parts of Ontario.
