Invitation to Submit a Proposal to conduct the Ontario Region First Nations Addictions Service Needs Assessment

The Chiefs of Ontario requires professional services with respect to conducting an Ontario Regional First Nations Addictions Service Needs Assessment. You are invited to submit a proposal for the provision of these services. Attached is a “Statement of Work” that provides detail on the work to be performed.Please note the following:

– The work is to commence by August, 2008 and be completed no later than March 2009.
– Your written response to submit a proposal is due no later than July 16, 2008

Responses shall include:

– A statement of understanding of the work and a proposed approach to the work (together not exceeding 5 pages in length), and a summary of at least two(2) projects successfully completed by your company that are of a similar size and complexity as that described in the “Statement of Work”;
– The name(s) and credentials of the person(s) proposed to perform the work;
– Hourly rates and daily rates for each proposed person(s), and an estimated contract total.
