Your Experience Counts: The 2025 NWT Patient Experience Questionnaire

The Department of Health and Social Services (HSS) has opened the 2025 Patient Experience Questionnaire to the public. The questionnaire is available to all northerners, in print and online.

The questionnaire has been designed to help the Health and Social Services system to better understand the quality of care provided to Northerners when receiving care in the Northwest Territories. Responses will help HSS improve services and monitor progress over time.

NWT residents who have accessed healthcare services in the last 12 months are encouraged to participate by completing the confidential questionnaire before November 10, 2025. It can be completed at one of the following places:

• Online
• Your local hospital or health center will have physical copies of the Questionnaire, or they can be requested by mail.

Interpretation services for Indigenous languages will be available by request at health centres, and through the GNWT’s active offer.
