Health Policy Forum 2nd Notice
Please be advised, the Chiefs of Ontario is holding a Health Planning Forum in Toronto to be held on April 22nd, 23rd and 24th, 2008. This Forum will be held at the Sheraton Centre Hotel, 123 Queen Street, West, Toronto, ON.
The Chiefs of Ontario are inviting 2 reps per First Nation, the Chief or Proxy and the Health Portfolio Holder of the community. We will be covering rooms for this Forum. A continental breakfast and lunch will be provided for the first two days, and a hot breakfast on the last day with mid morning snacks.All representatives will be responsible for their own travel arrangements. Attached is the Registration Form to be filled out and sent back as soon as possible and a sample Proxy Letter for your use.
